COMBINED SYNOPSIS/SOLICITATION.
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.


The solicitation number 12FPC119Q0005 is being issued as a Request for Quotation (RFQ) and is being conducted as a 100% Total Small Business Set-Aside. The Small Business size standard is based on the North American Industry Classification System (NAICS) code 562910 with a size standard of $20.5 million average annual income. This requirement will be awarded as a commercial item, fixed price purchase order under the procedures at FAR Part 12.


PROJECT DESCRIPTION. The USDA Farm Services Agency (GSA) requires document restoration services for paper records that were damaged by flood water. And for the water damaged paper records where document restoration is not possible/practical, FSA will require secure document destruction services for those paper records. (See Attachment 1, Performance Work Statement (PWS) for further details).


PROJECT DURATION. Contract award through completion, within 90 calendar days.


SUMMARY. FSA requires a contractor to perform document restoration services for approximately 36 cubic feet of paper records that were damaged by flood water. In addition, can also perform secure document destruction of paper records. The water damaged paper records are currently stored in the FSA facility located at 501 N. Main Street, Danbury, NC 27016. (See paragraph 4.0 REQUIREMENTS of the PWS).


Contractor shall furnish all personnel/management (i.e., labor), materials, services, equipment, tools, storage space, and insurance required to remove water damaged paper records from the FSA facility, complete the document restoration/document destruction. (See paragraph 7.0 CONTRACTOR PERSONNEL qualifications).

PREPARATION/SUBMISSION OF OFFERS
Addendum to 52.212-1, Modify (b): Submission of Offers to read as follows: Quotes shall be submitted on COMPANY LETTERHEAD, SIGNED, DATED a Pricing Schedule, and sent via email to Danny Mandell, Contracting Officer, email: Danny.Mandell@wdc.usda.gov, no later than April 16, 2019, 2:00 PM Central Daylight Time (CDT). The offeror shall provide the NRCS with a Firm Fixed Price quote that is in accordance with this solicitation. Pricing Schedule, contractor shall break down their pricing schedule, by description of services, quantity (boxes, materials), unit price (or cubic feet), total price for each category of services, (See paragraph 4.0 REQUIREMENTS of the PWS).


Potential Quoters are reminded to submit all the required documents by the due date, in addition, provide copy of their active SAM registration to be considered for award of a federal contract. Failure to provide the above requirements will make their offer non-responsive.

Potential Quoters are advised that they are responsible for obtaining and acknowledging any and all amendments to the solicitation. It is the Quoters responsibility to view the FedBizOpps website at www.fbo.gov, daily as no other notification of amendments will be provided on any revisions to this announcement or other notices.
