Develop Community Health Center Plan

County is soliciting a Statement of Qualifications from consultant agencies to aid County in the development of a comprehensive community health center. The objective is to select a highly-qualified consultant to provide guidance and shape strategic direction in order to achieve goals, objectives, and tasks.

Consultant Agencies would provide the following services:
Facilitate meetings to engage community and build partnerships
Conduct a Needs Assessment
Draft a Health Center Services Delivery Model
Assist County to identify and develop resources
