The Department of Human Services (DHS), Division of Developmental Disabilities (DDD), is seeking qualified vendors to assemble interdisciplinary teams of behavioral, medical, and other professionals within the developmental disabilities field that will be required to deliver, upon referral, coordinated services and supports. These services will be provided to individuals with developmental disabilities who are experiencing acute behavioral and/or medical conditions that result in chronic distress, despite previous attempts to address issues, which without intervention, could lead to displacement from current living environments. The teams will serve one or more of three geographic areas of the State: the Metro Chicago Area, the Upstate Area, and the Downstate Area. (Proposals will be reviewed separately in the context of each geographic area.) Each interdisciplinary team must have a commitment to and the capacity for making telephone contact and consultation with the service provider or family, as applicable, within 12 hours of referral by the DDD Central Office and for delivering services onsite within 48 hours of the referral. Successful vendors will be required to work collaboratively with each other, direct service providers, and service coordinators to ensure the best outcomes for the individuals referred.

It is important to note that the teams are not intended to take the place of emergency medical and psychiatric services, which are available to the general public. The teams may liaison with emergency room services as necessary, but this initiative is not an emergency referral system.
