This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. The Portland AREA's , Portland, Oregon, Western Oregon Service Unit Indian Health Service has the following request for creating a positive youth development program for health, nutrition and fitness program.
LOCATION: The Chemawa Indian High School is at 3750 Chemawa Road, NE, Salem, Oregon.
This is a Small Business Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. There is no solicitation available at this time. We are seeking sources in accordance with 25 USC 14, Buy Indian Act with preference to be given to Indian Small Business Economic Enterprises (ISBEE).
The purpose of this notice is to obtain information regarding: (1) the availability and capability of all qualified native small businesses; (2) whether they are small businesses; HUBZone small businesses; service-disabled, veteran owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification according to the North American Industry Classification System, 624190 code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice.
Background:
The Western Oregon Service unit is seeking capability statements from small business organizations (NAICS 624190) ; small business size standard is $11M).
This Sources Sought is for the PURPOSE

To provide PYD Health, Nutrition, and Fitness Programs during the school year. The purpose of the work is to improve the overall mental and physical health status of the CHS eligible Native American/Alaska Native boarding school patient population served. Most contacts are with patients receiving care at the Chemawa Bureau of Indian Education under the operating parameters of the IHS/BIE Memorandum of Understanding (MOU). The primary purpose of direct patient contacts is to provide a comprehensive array of culturally relevant public health prevention Positive Youth Development (PYD) Health, Nutrition, and Fitness Programs that reduce risk factors facing American Indian and Alaska Native Youth and promote resiliency,health,wellness for youth that attend the Chemawa Indian High School.
support and services, statistical and data analysis, and final clinical study report preparation and manuscript preparation. It is anticipated that 4-6 trials will be completed during the life of the contract.
Project Requirements Include:
(PYD) Health, Nutrition, and Fitness Programs promote physical and mental well-being through learning how to eat right, importance of physical fitness supporting mental fitness, being active and practicing good hygiene, and basic health principles and practices that can support life- long wellness, reduce adverse childhood event risk factors, and promote social well-being. Secondary program focus is on developing and strengthening communication and problem-solving skills, emotional regulation, conflict resolution, help seeking and coping skills directed at reducing risk and promoting resiliency in addressing alcohol, tobacco and drug use prevention, sexually transmitted disease, sexual health, bullying prevention, promotion of mental health, and suicide prevention. PYD - Health, Nutrition, and Fitness Programs along the public health continuum have been proven to support both physical, mental health, and human development needs including both physical and behavioral health wellness. This may include and not be limited to: Sexual Health, Intimate Partner Violence, Adverse Childhood Events/PTSD, substance abuse, eating disorders, diabetes, depression, anxiety, relationship problems and communication needs. PYD Health, Nutrition, and Fitness Programs will focus on both physical and emotional well-being, facilitating positive peer relationships and supports', learning that fosters self-esteem and self-worth, educational, physical, and/or psychological goals aligned with Western Oregon Service Unit Public Health and Behavioral Health Service Delivery priorities. Programs will be provided to promote life-long wellness, resiliency, and positive social skills building within the social and natural environment. All designated providers may be required to act within an integrated treatment team, support care coordination functions, maintain CNACI clearance, and secure E.H.R access as needed to document services, and support quality management practices that document and monitor program outcomes.


Information Requested:
1. Experience: An outline of previous work specific to the requirements and tasks listed above;
2. Personnel: Name, professional qualifications and specific experience of scientist and/or technical personnel who may be assigned as principal investigator and other key positions; and
3. Facilities: Availability and description of facilities and equipment required to conduct the analyses. Where was previous work performed; project name and dollar value; references.
4. Other: Any other specific and relevant information that would improve the Government's consideration and evaluation of the information presented is desirable.



The Capability Statement Must Demonstrate the Ability to:
For the Small business concerns that possess the capabilities necessary to undertake the efforts should submit complete documentation of their capabilities to the Contracting Officer. This capability statement should include: 1) the total number of employees, 2) the professional qualifications of scientists, medical experts, and technical personnel as they relate to the requirements, 3) a description of general and specific facilities and equipment available, including computer equipment and software, and 4) any other specific and relevant information that would improve the Government's consideration and evaluation of the information presented.


Further, Past Performance documentation, which should include, but not be limited to, a minimum of two (2) contracts performed for either Government or commercial organizations, shall be provided. References shall include for each contract: names, titles, contract number, total price or cost, telephone numbers of government Contracting Officer Representative and Contracting Officers.


Information Submission Instructions:


The offeror shall provide an electronic version only of their response to this notice. Responses shall include the organization's name, address, point of contact, size of business pursuant according to the North American Industrial Classification Code (NAICS) and must address all the technical information requested.
The page limit is 15 pages using a font size 10 or larger. Responses will assist the Government in selecting the appropriate acquisition mechanism.


Additionally, the response must provide answers to the following questions:
1. Is your organization a small business under NAICS code 624190?
2. Does your firm qualify as a small disadvantaged small business?
3. If disadvantaged, is your firm certified under section 8(a) of the Small Business Act?
4. Are you a certified HUB Zone firm?
5. Are you a woman-owned or operated business?
6. Are you a certified Service-Disabled Veteran Owned or Veteran Owned business?
7. Are you a certified Native American owned business?



This notice is for information and planning purposes only and does not commit the Government to any contractual agreement. This is not a Request for Proposals. The Government DOES NOT intend to award a contract based on responses under this announcement. Interested parties shall not be reimbursed for costs associated with preparation of their responses. Any proprietary information should be so marked. Interested organizations presenting a Capability Statement in response to this Small Business Sources Sought Synopsis must identify their size status.
Indian Economic Enterprise Representation.


To be considered for an IHS contract awarded under the Buy Indian Act, an offeror must represent that it meets the definition of "Indian economic enterprise" in response to a specific solicitation set-aside in accordance with this IHM.
Insert the following provision in all written IEE set-aside solicitations for supplies, services, or covered construction, regardless of dollar value:
INDIAN ECONOMIC ENTERPRISE REPRESENTATION
The offeror represents as part of its offer that it [ ] does [ ] does not meet the definition of Indian economic enterprise as defined in IHM 5-5.6.F.1.
Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA).
All information submissions shall be received no later than 3:00 pm Pacific time on June 29, 2018. After review of the responses to this announcement, the Government intends to proceed with the acquisition and a subsequent solicitation may be published or before July 3, 2018
All capability statements sent in response to this Small Business Sources Sought notice must be submitted electronically (via email) to the Point of Contact below in MS Word or Adobe Portable Document Format (PDF). The subject line must specify the Sources Sought Number. Electronically submitted capability statements are due no later than 3:00 PM (Pacific Standard Time) fifteen (15) calendar days (including date of submission to Fedbizopps) after posting of the Small Business Sources Sought Notice. CAPABILITY STATEMENTS RECEIVED AFTER THIS DATE AND TIME WILL NOT BE CONSIDERED.
