Custom Snap Out Forms

The intent of this Invitation for Bid is to establish a TERM CONTRACT to provide for the printing of Custom Snap Out Forms for use on various types of equipment by All State Agencies and Cooperative Purchasing Participants. Snap Out forms shall include those multiple pari forms with a stub that detaches from the main body of the form. Forms will be printed utilizing carbonless paper. Forms may be individual set, sets padded on chipboard or sets bound into a book. The forms will be priced by the stub size of the book.
