Crownpoint Ground Ambulance Transportation Services
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Navajo Area Indian Health Service, Crownpoint Health Care Facility is requesting for proposals from interested offerors to provide Ground Ambulance Transportation. It is requested that interested parties review the enclosed RFQ 18-245-SOL-00014 instructions and provide this office with a proposal, utilizing the information contained therein. The Government contemplates award of a requirements type firm fixed price contract resulting from this solicitation.
Completion of Solicitation Requirements:
Ensure all forms and additional information are completed in their entirety. Verify calculations before signing and submitting the proposal.
Offeror's proposal shall be prepared and submitted in accordance with Paragraph E.1 52.212-1 Instructions to Offeror Commercial Items. This paragraph shall stipulate the submission of all required documents, certifications, etc. to determine your responsibility of providing the required services. All proposals must be received in the Department of Acquisitions and Contracts by the Due Date listed.
