The Department of Veterans Affairs, Network Contracting Office 4, is conducting a market survey to identify potential sources to perform a firm fixed price contract, to include a base year and four possible option years, for Courier Transportation Services in Altoona, PA, as follows:

GENERAL:

General Scope: The successful offeror shall provide all vehicles, gasoline, insurance, personnel, directional materials necessary to accomplish daily (Mon Fri excluding Federal Holidays) round trip courier services between the James E. Van Zandt VA Medical Center (VAMC) and five Community-Based Outpatient Clinics (CBOCs) in Pennsylvania.

Place of Performance All daily round trips will start and end at the James E. Van Zandt VAMC.

Period of Performance- The contract shall include a one-year base period of performance, and four possible one-year option periods, to be exercised at the discretion of the government.

QUALIFICATIONS (not comprehensive):

Staff/Facility

The Contractor shall be an established business with at least 1-year experience in providing courier services to either the Federal Government or Civilian Agency.
The Contractor shall meet all requirements of Federal, State and City code regarding operation of this type of business in transporting items listed in this statement of work (SOW).
The Contractor s Personnel should be trained in appropriate safety packaging procedures suitable to items, quantities, distance being transported.
Contractor s personnel shall be licensed in accordance with the federal, state, and local transportation authorities, to include any special licensure/permission required for the transporting of the aforementioned items. Contractor shall provide copies of all personnel driver s licenses upon proposal.
Vehicles used in the performance of this contract shall be registered and meet all minimum vehicle requirements as mandated by the Department of Transportation, to include Insurance. Contractor shall provide copies of insurance and registration for each vehicle utilized upon proposal.

3. CONTRACTOR RESPONSIBILITIES (not comprehensive):
3.1 The courier service shall provide round trip delivery services between the James E Van Zandt VAMC and (5) CBOCs. Items shall be picked up at the James E. Van Zandt VAMC. All items picked up shall arrive at the (5) CBOCs no later than 8:00am to ensure available day of for patient care. Courier service shall pick up items from each of the (5) CBOCs that are to be returned to the James E. Van Zandt VAMC.

3.2 Items to be transported to and from the James E. Van Zandt VAMC and CBOCs include, but are not limited to:
Office supplies and equipment
Medical supplies, equipment, and Durable Medical Equipment (DME) (including prosthetics, wheelchairs, etc.)
Brochures, workbooks, education materials, etc.
Interoffice mail
Peripheral computer equipment
Supplement samples
Snacks for diabetics (non-perishable)
Signage
Oxygen Cylinders
Vaccines and pharmaceutical items

3.3 All items will be stored in Room [TBD] the evening prior so available for pick up the next morning. Items will be placed on a cart designated for each of the (5) CBOCs. All mail will be properly addressed and secured. All pharmaceuticals and vaccines will be properly stored/packaged in either a locked red plastic box or in a Styrofoam cooler with ice packs.

3.4 Upon return from CBOCs, all items, except for Pharmacy items, shall be delivered to Room [TBD] and placed in designated bin or area for VA staff to pick up. All red bins, coolers, and items addressed to Pharmacy shall be delivered to the Consultation Window of the Pharmacy on the 1st Floor of the Medical Center.

3.5 The Pharmaceuticals and vaccines in the red bins, as well as the coolers shall not be left in a vehicle (other than during transport) and shall not exceed 72 degrees or go below 45 degrees.

3.6 Drivers shall not make non-emergent stops, for any reason, during the transport of items under this contract.

3.7 The Contractor shall contact the COR by 6:30am the day of when weather indicates adjustment or cancellation of scheduled pick up/deliveries.

3.8 Under no circumstances shall any person other than the courier driver or another employee of the contracting agency ride in a vehicle. Items being transported will be handled by a trained courier driver.

3.9 While on VA grounds, the contracted courier driver shall secure vehicles when left unattended. If a vehicle must be left unattended with the motor running, a second set of keys will be necessary, so that vehicle can be securely locked.

Accordingly, prospective contractors will be required at time of solicitation response to certify to the applicable limitation on subcontracting requirements in 13 CFR 125.6, with reference to the appropriate set-aside. Please include in the response to this sources sought your ability or inability to certify to the clause applicable to your organization.
