This document is a Request for Proposals (REQUEST) issued on behalf of the Utah State Department of Human Services, Division of Substance Abuse and Mental Health (DHS/DSAMH or Requesting Agency). The purpose of this REQUEST is to identify and contract with qualified Offerors to conduct court ordered FORENSIC EVALUATIONS. This REQUEST is designed to provide interested Offerors with sufficient information to prepare a Response that will meet the minimum requirements of the REQUEST. However, it is not intended to limit or exclude content or data deemed relevant or essential by the Offeror. In fact, Offerors are encouraged to expand upon the specifications in the REQUEST to evidence their service capability under any agreement.
