Comprehensive Youth Outreach and Early Intervention

Health Services, hereafter referred to as "County", is seeking proposals from interested and qualified non-profit organizations, educational entities, or faith-based organizations to provide a Comprehensive Youth Outreach and Early Intervention Program.
The contract will be a cost reimbursement contract for mental health prevention and early intervention services. The program will provide community outreach and case management to at-risk youth and young adults ages 16-24.
One (1) contract will be awarded. Funding of up to $135,000 is available the first eight (8) months of this program, (anticipated to be November 1, 2011 - June 30, 2012). The contract awarded may be extended for up to two (2) additional one-year terms, at a maximum of $200,000 per year, at the discretion of the County, based upon the availability of funds and Contractor performance. The maximum length of funding for the program will be three (3) years.
