Comprehensive Pharmacy Services - Hocking Correctional Facility

The term of this contract is from September 1, 2008 to June 30, 2009 with two optional two-year
renewals by mutual agreement.
Requirements One FTE equals 40 hours service per week
1 FTE Pharmacist Services 40 hours per week
1 FTE Pharmacy Technicians 40hours per week
Reconciliation of the staffing plan will be completed on a quarterly basis with an allowance of 95%
of total FTE hours.
*Note: Any changes to the contractors proposed staffing plan after the award of the bid must be
approved by the HCF Health Care Administrator and ODRC Bureau of Medical Services, prior to
implementing any staffing changes.
