The State is seeking an organization or consultant to create and develop/educate/market to users of this grant. Money Follows the Person is a five year demonstration grant whose application is targeted at residents residing in a nursing facilities throughout the state who face barriers to discharge related to lack of initial transition supports and other services necessary to address their complex needs The bidder must have knowledge and experience working with local and state organizations who have a role in Options Counseling and Referral and Intake services; ADA accessibility standards and general knowledge of individuals with disabilities and older adults.
