Community Development Specialist Housing & Transportation
The State is seeking an organization or consultant to create a real-time on-line housing and transportation inventory. Money Follows the Person is a five year demonstration grant whose application is targeted at residents residing in a nursing facilities throughout the state who face barriers to discharge related to lack of initial transition supports and other services necessary to address their complex needs. The bidder must have knowledge and experience working with local and State Housing Authorities and landlords; ADA accessibility standards and general knowledge of individuals with disabilities and older adults.
