Communities of Excellence Program
The selected contractor will be responsible for conducting Communities of Excellence in Nutrition, Physical Activity and Obesity Prevention (CX3) food environment assessments in the following four (4) neighborhoods: Boyes Hot Springs (Sonoma Valley), Roseland (Santa Rosa), Kawana Springs/South Park (Santa Rosa), and Downtown Santa Rosa. Activities include:
1) Train surveyors to conduct CX3 food environment assessments, which should include the involvement of community members (e.g., adults or youth) from the neighborhoods, as well as community leaders (e.g., faith, city engineers).
2) Oversee field work, utilizing all appropriate CX3 surveys and tools, and organizing data.
3) Based on CX3 findings, prioritize identified problem areas with stakeholders, community groups, and organizations.
4) Assist with the creation of a CX3 strategic community- focused action plan.
