This request is for a primary care practice to provide primary care services during normal business hours. The primary provider will refer these patients to the agency for all specialty, lab, radiology, and inpatient services. Reimbursement shall be made on a per month/per member basis. The clinic will be required to meet the quality care measures imposed by the plan.
Vendor must have at least three (3) years experience with preparation of Corrective Action Plans related to license regulations identified via regulatory surveys that require a time limited response.
