Certified Registered Nurse Anesthetists (CRNAs) at a Department of Defense Level 1 Trauma Center
This notice is a final solicitation to seek qualified 8 (A) Small Business firms interested in submitting potential Certified Registered Nurse Anesthetists (CRNAs) at a Department of Defense Level 1 Trauma Center, under the North American Classification System (NAICS) Code of 621399, Office of All Other Miscellaneous Health Practitioners.
The Small Business Administration's size standard for NAICS 621399 is $7,500,000.00.
The Department of Anesthesiology, San Antonio Military Medical Center (SAMMC), Joint Base-San Antonio, Fort Sam Houston, Texas, has a requirement for 25 CRNAs.
Offerors shall submit copies of their proposal via email to arlisa.t.harley.civ@mail.mil. Hard copies of the proposal are not required and will not be accepted. Offerors shall include a completed and signed SF1449 with their proposal. Proposals shall arrive no later than 7 August 2018, 12:00 PM CST. Any proposals submitted after the deadline will be considered untimely and ineligible for award. Note: Page 1 of the SF 1449 with signature of a corporate officer authorized to negotiate for the company in Block 30a, and acknowledgement of all amendments, (SF 30) with signature in Block 15B are required.
Questions regarding this requirement shall be submitted in writing using the attached template (Attachment 4) to the Contracting Officer (CO), Barbara Nelson at barbara.c.nelson8.civ@mail.mil, and copy to Contract Specialists, Arlisa Harley at arlisa.t.harley.civ@mail.mil on or before 19 July 2018. Questions submitted after the question due date will not be accepted.
