The purpose of Amendment 0001 is to do the following:

1.) Extend the proposal due date from April 9, 2019 to April 30, 2019;

2.) Extend the contract award date from July 16, 2019 to July
23, 2019;

3.) Extend the contract period of performance from July 16,
2019 through July 15, 2024 to July 23, 2019 through July 22, 2024;

4.) Respond to the Offeror's Questions;

5.) Add Section J-5 Federal Aviation Administration (FAA) Order 1370.120 Section 508 Accessibility Policy and;

6.) Provide changes to Sections: C, F, H L, L-2, and M (see highlights).

The Federal Aviation Administration (FAA) has a requirement for a commercially available off-the- shelf Safety Data Sheet (SDS)Database. The FAA must maintain a list of hazardous chemicals and the corresponding current Safety Data Sheet (SDS) at every FAA site/location/facility where hazardous chemicals are utilized and stored.

Scope of Effort:
This Screening Information Request/Request for Offer is for a Safety Data Sheet (SDS) Database.

Contract Type and Period of Performance:
This is a total small business set aside. The North American Industry Classification System (NAICS) codes for this effort are: 511210, Software Publishers; 541519, Other Computer Related Services, and 541511, Custom Computer Programming Services. The small business size standard dollar threshold is $38.5M.

(b)    Time and Date - Proposals must be received by the FAA at the above location no later than 4:00 pm Eastern Time on 4/16/2019. Offerors must request receipts for proposals if one is desired. All SIR Amendments issued, if any, must be signed on the cover page Solicitation, Offer and Award (Standard Form 33) and submitted with the Offerors proposal.

(c)    Signed Originals - Offerors must provide one signed original and all required copies.

(d)    Proposal Submission - Offerors assume full responsibility of ensuring that proposals are received at the place and by the date and time specified above. Facsimile or E-Mail submittals of proposals will not be accepted. Proposals may be hand-delivered or mailed by commercial delivery or US Mail.

Questions:
Questions pertaining to this SIR must be submitted in written form through email to Stephen Young, Contracting Officer, at Stephen.young@faa.gov no later than 4:00 p.m. Eastern Time on, March 5, 2019.

Questions and responses that clarify the SIR may be provided to all Offerors; however the source of the questions will not be identified. The FAA plans to publish responses to questions on FAA Contract Opportunities.

Amendment#: 0001 does the following:
1.) The proposal due date is being changed from April 9, 2019 to April 16, 2019.

2.) The contract award date is being changed from July 16, 2019 to July 23, 2019.

3.) Section L sub section L.17.1.1 had the title changed from Technical Capability to Product Specifications.

4.) Minor edits were made to Section L such as sentence revisions to make things clearer.

5.) The proposal due date was changed in Attachment L.2, Past Performance Questionnaire (PPQ) from April 9, 2019 to April 16, 2019.

6.) Section M was modified to include Risk Assessment.

7.) Section C was modified to have the Table of Contents removed from the top of the page.

8.) This amendment answers the vendors questions.

9.) This amendment changes the period of performance date from July 16, 2019 through July 15, 2024 to July 23, 2019 through July 22, 2024.
