Case Management and Nutrition Services for Older Adults
County is seeking a contractor to:
(a) Provide case management services to older adults.
(b) Provide nutrition site management at a noon-hour
congregate meal site for older adults Mondays through Fridays at a
meal site location determined by the contractor and approved by the
County.
(c) As part of nutrition site management, arrange the provision
of home delivered meals Mondays through Fridays to home bound
older adults (and people with disabilities under the age of 60 when the
individual has outside funding to cover the full cost of the meal).
