Case Management Services
Residents of the facility require assistance in accessing mainstream services and acquiring household necessities. Basic needs include counseling, medical assistance, employment training and continuing education to ensure there is not a reoccurrence of homelessness. Following initial assessment, a supportive services plan is developed and regular meetings ensure the resident meets specific goals. It is anticipated that at least 50 clients will be served through this grant funded program.
REQUIREMENTS
The Department requires an experienced firm to provide case management services at the facility. Services will include an assessment of resident needs, development of a service plan, the establishment of supportive services, on-going assessment of client needs, as well as the preparation and maintenance of client files, monthly reporting of client services and annual reporting of residents progress and outcomes.
