Case Management & Transportation For Substance Abuse Treatment Program
The Department is issuing a Request for Proposals with the intent to fund an outcome-based system of enhanced Transportation and Case Management services for substance abuse treatment programs serving women with children, pregnant women and/or youth and their families. The Case Management and Transportation Services programs are designed for low-income persons with no other funding source for these services. The case management and transportation programs deliver services for persons in Outpatient Treatment that are designed to help them handle aspects of their lives that are not necessarily related to an addiction disorder but that might impact whether the patient remains in treatment or has successful treatment outcomes.
