Caregiver Program Investigator Services
The purpose of this document is to provide interested parties with information to enable them to prepare and submit a bid to the Department of Health Services (DHS), Division of Quality Assurance (DQA) for performing state-required investigations. Investigations include those of allegations of abuse, neglect and misappropriation complaints filed against non-credentialed persons. Additionally, allegations that a facility had prior knowledge of the date scheduled for an unannounced survey may also be required.
