This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement.
Services Required: The Department of Veterans Affairs (VA), Veterans Health Administration (VHA) Tennessee Valley Healthcare System (TVHS) requires the following services be provided in a private hospital, office or clinic environment to Veterans, primarily residing within the TVHS coverage area, including seven contract clinics in the following areas: Stewart County, TN; Maury County, TN; McMinnville, TN; Roane County, TN; Cookeville, TN; Bowling Green, KY; and Hopkinsville, KY. Contracts are currently in place for these seven Community Based Outpatient Clinics (CBOCs). It is anticipated that it will take approximately 12-18 months to complete the procurement process to replace these contracts.
Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method.
NAICS code: 621498 and sized standard is $20.5MIL.
NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry https://www.va.gov/osdbu/. Additionally, all Offerors responding to a solicitation must have an active registration in the System for Award Management (SAM) at the time of proposal submission.

Project requirements: Primary Care Community Based Outpatient Clinic (CBOC): Offer both medical (physically on-site) and mental health care (physically on-site) and may offer support services such as pharmacy, laboratory, and x-ray as required herein below. Primary Care CBOCs are required to provide both primary care and mental health services. Specialty care not provided on-site, shall be available through referral or telehealth. To facilitate provision of TeleMental Health medicated care, the CBOC site will have one (1) room reserved exclusively for TeleMental Health treatment. A Primary care CBOC often provides home-based primary care (HBPC) and home telehealth to the population it serves to meet the primary care and mental health needs of Veterans who have difficulty accessing clinic-based care. These services may also use VA Video Connect to see their patients in the patient s home. Primary care in VA includes both medical and mental health care services, as they are inseparable in providing personalized, proactive, patient-centered health care. Primary Care Requirements. A point of service is said to provide primary care services if the site registers more than 500 primary care encounters within the primary care stop class within a given fiscal year. Mental Health Requirements. A point of service is said to provide mental health services if the site registers more than 500 mental health encounters within a single mental health clinic stop class within a given fiscal year. Mental health services will be provided on-site by contractor. Contractor may supplement Mental health services via telehealth, if approved by the Chief, MH Service Line, Mental Health COR, and Contracting Officer.

Stewart County, TN, approximately 83 miles from the TVHS Nashville Campus, and 132 miles from the TVHS Murfreesboro Campus;
Cookeville, TN, approximately 85 miles from the TVHS Nashville Campus, and 116 miles from the TVHS Murfreesboro Campus;
Roane County, TN, approximately 142 miles from the TVHS Nashville Campus, and 125 miles from the TVHS Murfreesboro Campus;
McMinnville, TN, approximately 78 miles from the TVHS Nashville Campus, and 40 miles from the TVHS Murfreesboro Campus;
Maury County, TN, approximately 43 miles from the TVHS Nashville Campus, and 52 miles from the TVHS Murfreesboro Campus;
Bowling Green, KY, approximately 65 miles from the TVHS Nashville Campus, and 89 miles from the TVHS Murfreesboro Campus; and
Hopkinsville, KY, approximately 72 miles from the TVHS Nashville Campus, and 111 miles from the TVHS Murfreesboro Campus.
Anticipated period of performance: Period of performance shall be for a base period of one-year with 6 one-year option periods.
Capability statement /information sought: Response to this announcement shall not to exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide a product and related services; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc,) pursuant to the applicable NAICS code; and (4) Information should be provided electronically in a Microsoft Word or Adobe PDF format. Provide any other information that may be helpful in developing or finalizing the acquisition requirements.
