SOLICITATION MUST BE COMPLETED IN ITS ENTIRETY AND RETURNED IN ORDER TO BE CONSIDERED RESPONSIVE. YOU MUST FILL OUT THE FOLLOWING.
1. Read box 28 and sign page 1 boxes 30 a, b, and c.
2. Provide your cage code or duns number.
3. Please quote FOB Destination Bremerton, WA.
4. You must fill out a unit price for each line item that corresponds to what is requested and that includes all costs (shipping ect.).
5. Annotate the lead time.
6. Annotate the name of the manufacturer, if they are large or small business and the city/state/country of manufacturing.
