COMPUTER OUTPUT MICROFILMING SERVICES
The Illinois Department of Employment Security is soliciting this Invitation for Bid to vendors interested in providing Computer Output Microfilm services of 16mm roll microfilm, 105 mm microfiche and CDs for IDES Central Office at 33 South State Street, Chicago Illinois. It is required that all Contractors be registered with the State of Illinois, have a DHR Number, and not be delinquent in Unemployment Insurance Taxes. No MBE/FBE goals have been established for this project; however, we do encourage using MBE/FBE Subcontractors and suppliers to perform the work. If the Contractor intends to use MBE/FBE Subcontractors or suppliers, submit a list of proposed MBE/FBE Subcontractors or suppliers in the space provided on the Bid Forms.
