COMMUNITITY BASED ASSESSMENT (CBA) SERVICES
The Department of Employment, Training and Rehabilitation (DETR) is currently seeking vendors that can respond favorably to this request for qualification for Community Based Assessment (CBA) Services. These services shall include:Community based assessments;Situational assessments; and Job coaching. Vendors that successfully qualify may be offered a contract with the State of Nevada for these services. Any resultant contracts shall commence on July 1, 2008 and be for a term of one year with the possibility of extensions if mutually agreed upon in writing and in the best interest of DETR and the State of Nevada. Vendor agrees to this fee schedule that pays a $65.00 per day for the supervisory fee when the consumer works for 4 or more hours a day and up to $37.50 per day for supervisory fee when the consumer works less than 4 hours per day. Provide costs for placing a consumer on the CRP payroll. Division will reimburse Vendor for consumers pay at minimum wage and appropriate workers compensation. Vendor will provided fee schedule for providing this service. Division reserves the right to negotiate such costs and is not obligated to enter into a contract if the Division feels the costs are excessive.
Pricing for Job Coaching. For Job Coaching, the Rehabilitation Division pays according to the fee schedule in Attachment F, Pricing/Fee Schedule. In submitting a proposal the vendor agrees to this fee schedule that pays $23.50 per hour.
