CHRONIC DISEASE MANAGEMENT SYSTEM
The major objectives in implementing the CDMS are to:
* Provide a stable chronic disease management solution that integrates with existing systems
at CCHS.
*Provide disease management and prevention capabilities that facilitate care management for the CCHS population.
* Share appropriate clinical data with disease management and quality improvement collaboratives.
* Comply with payer, state, and federal reporting requirements.
* Streamline and standardize administrative and clinical processes.
* Make more informed clinical decisions as a result of having access to complete and relevant
patient information and decision-support functionality at all locations.
A single vendor must be able to provide the solution in its entirety. This vendor may choose to
partner with third parties to provide the solution; however, the respondent is entirely responsible for
meeting the requirements as defined in this request. The successful respondent will be considered
the prime contractor and, as such, will be held solely responsible for the agreed-upon solution as
described in its response.
