FAR 52.212-1 - Instructions to Offerors - Commercial Items. Addendum.
General Information
This acquisition will be solicited and awarded in accordance with FAR Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures. This acquisition is solicited as a Small Business Set-Aside.
Quote submission instructions:
The overall quotation package shall consist of two (2) separate volumes, individually entitled as stated below:
Volume Title
Volume I Offer and Other Documents
Volume II Past Performance
Volume I - Offer and Other Documents
This volume shall contain the offerorB?s contractual documentation.
This volume consists of required representations, certifications, and acknowledgements, any other administrative information, and a summary of exceptions and deviations taken.
Although it incorporates them by reference, it does not physically include the other volumes. Volume 1 - Offer and Other Documents, shall include the following documents (in the order listed):
1) The SF 1449 with price printed in the space provided, and blocks 17a, 30a, 30b and 30c completed. The SF 1449 is to be executed fully (to include amendments when applicable) and used as the cover sheet (or first page) of Volume I - Offer and Other Documents. The person signing the form must have the authority to commit the offeror to all of the provisions of the RFQ, fully recognizing that the Government has the right, by terms of the solicitation, to make an award without further discussion if it so elects.
2) Offeror Representations, Certifications, and Other Statements fully executed as required pursuant FAR 52.212-3 - Offeror Representations and Certifications - Commercial Items. This provision is a part of the Representations and Certifications that are submitted and maintained on the System for Award Management (SAM) website. Offerors shall ensure that the Representations and Certifications are up to date in SAM. Offerors that do not have an active registration in the SAM system shall print a FULL TEXT copy of this provision, complete the required entries, and submit with their quote. Offerors that do not have an active SAM registration or do not submit their Representations and Certifications with their quote MAY be considered nonresponsive and their quote MAY NOT be evaluated. Potential awardees SHALL have an active SAM registration prior to award of purchase order.
3) Exceptions and Deviations taken to the model purchase order and/or solicitation requirements. Summary of Exceptions and Deviations taken on other Volumes.
Volume II - Past Performance
BLM is seeking to determine whether each offeror consistently delivers quality services, in a timely and cost effective manner with a reasonable degree of customer satisfaction. Where applicable, the evaluation of past performance will be conducted in accordance with FAR 13.106-2. The offeror shall provide Past Performance References as requested in Attachment 6. Please fill out as much information as you can. Past Performance references should be provided for three (3) recent (performed during the last five (5) years). References should reflect ability, experience and customer satisfaction for services similar to those listed in this solicitation..
Offerors are cautioned that in conducting the past performance evaluation, the Government may, at its discretion, use additional information obtained from other sources (such as official electronic past performance databases and/or other government and local agencies) as well as that provided in the offeror's quote. While the Government may elect to consider information obtained from other sources, the burden of providing complete information rests with the offeror.
Utilizing the Past Performance References submitted by the offeror and other sources of past performance information an evaluation will be conducted as outlined in FAR 52.212-2, Evaluation - Commercial Items, located in Attachment 4 of this solicitation.
Quote Delivery
Quotes shall be submitted via electronic mail to the below stated point of contacts. Any questions or concerns shall be addressed to the below individuals only. All questions must be in writing. Quotes are expected to conform to the solicitation provisions and be prepared in accordance with this section.

Expenses Related to Offeror Submissions
The offeror shall not be reimbursed for any costs incurred prior to the effective date of a purchase order. This Request for Quote (RFQ) does not commit the Government to pay any costs incurred in the submission of any quote or for the preparation thereof or to acquire or contract for any services.
Offerors are encouraged to read FAR Clause 52.212-1, Instructions to Offerors-Commercial (located in Attachment 3) and additional instructions to ensure compliance before quote submission.
