COMBINED SYNOPSIS/SOLICITATION
SERVICE MAINTENANCE AGREEMENT FOR FOUR CARESTREAM DRX REVOLUTION MOBILE X-RAY SYSTEMSLOCATED AT THE EAST ORANGE MEDICAL CAMPUS OF THE VA NEW JERSEY HEALTHCARE SYSTEM
(i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 and 13 Simplified Acquisitions, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
(ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number 36C24218Q0512.
(iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1.
(iv) This procurement is being issued as an open market unrestricted solicitation. The North American Industry Classification System (NAICS) code is 811219, Other Electronic and Precision Equipment Repair and Maintenance, with a small business size standard of $20.5 million.
(v) The Contractor shall provide pricing to provide service maintenance agreements for four Carestream DRX-Revolution mobile x-ray systems located at the East Orange Medical Campus of the VA New Jersey Healthcare System. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will be accomplished by using the prices offered for the last option period to determine the price for a 6-month option period, which will be added to the base and other option years to arrive at the total price. Evaluation of options will not obligate the Government to exercise the option(s). See attached document for pricing tables to be completed and submitted with the quote. The table below lists the timeframe of option periods with reference the statement of work (SOW):

(vi) Description of requirement (See attached Statement of Work (SOW)): The contractor shall provide all parts and labor identified in the SOW.

(vii) Awardee shall coordinate with the Contracting Officer s Representative (COR) prior to performance.

(viii) The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The following provisions are included as addenda to FAR 52.212-1:

(ix) Evaluation will be determined based on Best Value to the Government IAW FAR Subpart 13.106-1(a)(2). The government shall award this procurement to the vendor with the lowest priced quote that is technically acceptable and has acceptable, or neutral, past performance. Vendors with no past performance shall be rated as neutral.

PRICING

The Offeror shall complete and submit the table listed in section (v) of this combined synopsis and solicitation for pricing evaluation.


TECHNICAL

The Offeror shall provide capability statement that indicating that the vendor has the trained personnel, equipment, and track record of working experience in providing service support for Carestream DRX-Revolution mobile x-ray systems meeting all of the requirements identified in the scope of work.

The Offeror shall include documentation indicating that they are authorized to service Carestream DRX-Revolution mobile x-ray systems by the Original Equipment Manufacturer.




PAST PERFORMANCE

The Government shall request the contractor provide a list of contracts completed during the past three (3) years and all contracts and subcontracts currently in process for dental laboratory services. Include the following information for each contract:

Name and address of contracting activity
Name and telephone number of contract person
Dates of contract performance

The Government reserves the right to make award without discussions.

(x) The provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items applies to this acquisition. * All firms or individuals submitting a quote shall include a completed copy of this provision, OR have current Representations and Certifications in the System for Award Management (SAM - https://www.sam.gov). Representations, Certifications, and SAM must be current at award. IF NOT CURRENT AND/OR YOU DO NOT SUBMIT A COMPLETED COPY OF THIS PROVISION, YOUR QUOTE WILL BE CONSIDERED NON-RESPONSIVE.
