1. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued.

a. Project Title: Boiler Waterside Inspections
b. Description: VA Long Beach Healthcare System is seeking a contractor to provide annual and initial psychological assessments for current and potential police officers.
c. Project Location: VA Long Beach Healthcare System, 5901 East 7th Street, Long Beach, CA 90822
d. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26219Q0989.
e. Set Aside: This solicitation is issued as a 100% Service Disabled Veteran Owned Small Business set aside.
f. Applicable NAICS code: 238220
g. Small Business Size Standard: $15 Million
h. Type of Contract: Firm Fixed Price
i. Period of Performance: Base year and four (4) one-year options, beginning 8/1/2019 7/31/2020

2. Statement of Work

2.1 Contract Title. Annual Boiler Plant Waterside Inspection of Boiler Tubes on all Three (3) Simoneau D-Type, FX2 Boilers, (PREVENTIVE MAINTENANCE PROGRAM).

2.2. Background. Annual Waterside Inspections are used to determine the effectiveness of water treatment, maintain maintenance procedures, diagnose of all boiler operating troubles, overall general conditions of the boilers, and for compliance with the VHA CEOSH, SCAQMD, OSHA, and all other Regulatory Agencies.

2.3. Scope.

2.3.a. Contractor shall provide all labor, parts, materials, equipment proper PPE and Supervision required to perform Annual Boiler Plant Waterside inspection of Boiler Tubes on all Three (3) Boilers.
2.3.b. Contractor shall comply with all VA, OSHA, Industry Standards, Safety rules and regulations per VHA CEOSH.
2.3.c. Contractor shall check in with Larry Davis, Boiler Plant Supervisor Bldg-5, upon arrival at the VA Long Beach Healthcare System.
2.3.d. Contractor and Employees shall report to the COR, Keith Didrickson, for the issuing of Temporary ID Badges from the VA Police Department, that are to be worn at all times while on VA Property.
2.3.e. Supervisors must complete 30 Hour OSHA Construction Safety Training, per VHA Directive 2011-036.
2.3.f. Personnel working on the contract must complete 10 Hour OSHA Construction Safety Training, per VHA Directive 2011-036.
2.3.g. Contractor must possess a valid California C4 Contractors License.
2.3.h. Contractor and Employees must attend a 30 minute VA Safety briefing provided by the Safety Department.

2.4. Specific Tasks. Boiler Plant Annual Waterside inspection of Boiler Tubes for all three (3) Simoneau D-Type, FX2 Boilers.

2.4.a. Contractor shall open, clean and flush watersides of the Boilers. Clean and remove any deposits or scale accumulation in the bottom header.
2.4.b. Contractor shall open and clean low water cut-off assemblies.
2.4.c. Contractor shall flush and re-seal controls and equalizing piping.
2.4.d. Contractor shall check for damage to controls or floats and close up using new gaskets.
2.4.e. Contractor shall provide and install new sight glass valves and Pyrex red line sight glass on water column.
2.4.f. Contractor shall open burner for firebox access, inspect and clean firebox area.
2.4.g. Contractor shall visually inspect tubes from the firebox for evidence of leakage or overheating.
2.4.h. Contractor shall inspect refractory and firebox insulation.
2.4.i. Contractor shall seal and close fireside of Boiler using factory gaskets.
2.4.j. Contractor shall fill unit to proper level and inspect units for leaks, vent air from unit and system and re-install in accordance with recommended Manufacturers specifications and tolerances.
2.4.k. Contractor shall check flame Safeguard Control for pilot and main flame ignition.
2.4.l. Contractor shall check operation of blower motor and circuitry.
2.4.m. Contractor shall check and adjust all Boiler limits, pressuretrols and running interlocks.
2.4.n. Contractor shall check burner, pilot and main flame ignition.
2.4.o. Contractor shall check operation and adjust all linkages, or servos in accordance with Manufacturers/Factory specifications.
2.4.p. Contractor shall check operation of gas valves and vents.
2.4.q. Contractor shall Calibrate the burner for optimum air/fuel mixture for each fuel and firing to be fine-tuned and adjusted to its maximum efficiency level throughout the entire modulation range. Flue gas analysis and monitoring of temperature, O2, CO2, CO, NOx and excess air to be monitored.
2.4.r. Contractor shall perform combustion and efficiency testing, record all firing rates of burner capacity. Combustion efficiency will be calculated and recorded at each position.
2.4.s. Contractor will submit an engineering report upon completion of testing.
2.5. Performance Monitoring - Routine inspections by Facilities Personnel
2.6. Security Requirements. Vendor will not have access to patient records, data or VA computer systems
2.7. Place of Performance. Bldg. 5. VAMCLB.

3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-02, dated 5/6/2019. The following provisions and clauses apply to this solicitation and can be found in enclosure 2.

4. Submit written offers in accordance with the following instructions, as well as Addendum to FAR 52.212-1, Instruction to Offerors. Oral offers will not be accepted. Complete and submit enclosure 1 via email no later than 10am PDT Tuesday July 23, 2019 to peter.kim33@va.gov. Ensure the following are completed and/or included in the quote packet:
