The 10th Contracting Squadron, 8110 Industrial Blvd, USAF Academy, CO 80840 is contemplating an Award for Biomedical Hazardous Waste Disposal for a 1 year base period plus four (4) twelve month options. This is a commercial purchase using FAR Part 12 and Part 13 procedures. The following information, as required per FAR 12.603(c)(2) applies:
(I) This is a combined synopsis/solicitation for commercial items prepared in  accordance with FAR Part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued.
(II) The solicitation reference number is FA700019QA076.
(III) The solicitation document, incorporated provisions, and clauses are those in effect through the Federal Acquisition Circular 2019-05 effective 13 Aug 2019, and the Defense Federal Acquisition Regulation Supplement DPN 20191131 Edition effective 31 May 2019, and Air Force Federal Acquisition Regulation Supplement AFAC 2014-0421 effective 21 Apr 2014.
(IV) This acquisition is set-aside for Small Business under NAICS Code 562211. Business size standard for this NAICS is $41,500,000.00.
(V) Contract Line Item Number(s) CLINS:
PROVIDE PRICING FOR EACH CLIN WITH YOUR RESPONSE. QUOTES MUST BE SUBMITTED USING THE ATTACHED MICROSOFT EXCEL "BIOHAZARD PROPOSAL" SPREADSHEET. ALL INFORMATION MUST BE FILLED IN. SUBMISSIONS NOT USING THE SPREADSHEET WILL BE DISREGARDED UNLESS AGREED TO PRIOR TO CLOSING OF THIS SOLICITATION.
Contractor shall provide all labor, tools, supplies, and equipment to pick-up and dispose of medical waste generated by the 10th Medical Group in accordance with (IAW) the United States Air Force Academy (USAFA), Biohazard Waste Disposal, Performance Work Statement (PWS).
Estimated cost is for base (CLIN 0001), plus four (4) option years (OY).
- CLIN 0001, Base, Period of Performance (POP) is 1 October 2019 - 30 September 2020.
- CLIN 1001, Option Year One (OY1), POP: 1 October 2020 - 30 September 2021.
- CLIN 2001, Option Year Two (OY2), POP: 1 October 2021 - 30 September 2022.
- CLIN 3001, Option Year Three (OY3), POP: 1 October 2022 - 30 September 2023.
- CLIN 4001, Option Year Four (OY4), POP: 1 October 2023 - 30 September 2024.
(VI) Description of requirements: This is an acquisition being issued as a Request for Quotation (RFQ). Contractor will provide services in accordance with the attached Performance Work Statement (PWS).
(VII) Service shall be provided at the following locations and frequencies:
- Building 4104; Biohazard Waste Storage - once per week
- Building 2355; Cadet Medical Clinic - every other week
- Building 2355; Cadet Dental Clinic - every other week
- Building 2355, Room 2P469; Biology Department - once per month
- Building 5136; Community Center Health Clinic - once per month, pick-up of one (1) to two (2) sharps containers per month only
- Building 9037; Veterinary Clinic - once per month
- Building 2170; Sports Medicine complex- once per month
- Option to utilize emergency pick-up as needed. Emergency pick-up is typically not required during normal operations, but will be a maximum of 2 emergency pick-ups during each quarter. Any requests for emergency pick-up will occur during normal
operating hours.
- All pick-up times will be coordinated with the end customer.
- FOB: Destination
(VIII) The contractor shall provide the following containers for:
- Building 4104, Biohazard Waste Storage - Fifteen (15) 40-gallon containers
- Building 2355, Cadet Medical Clinic - One (1) 40-gallon container
- Building 2355, Dental Clinic - One (1) 40-gallon container
- Building 2355, Biology Department - Six (6) 40-gallon containers
- Building 5136, Community Center Clinic - no containers required; customer has sharps containers at this location
- Building 9037, Veterinarian Clinic - One (1) 40-gallon container
- Building 2170, Sports Medicine Department - One (1) 40-gallon container

QUOTES ARE DUE AT THE USAF Academy, COLORADO, NOT LATER THAN 10:00 A.M. MST ON 20 September 2019.
Addendum to FAR 52.212-1(b), Submission of Quotes:
Submit signed and dated quote via e-mail, Attn: Miles Montgomery, Contract Specialist, 8110 Industrial Drive, Suite 200, USAF Academy, CO 80840-2315. Quotes must be received no later than 10:00 am Mountain Standard time, 20 September 2019 to louis.montgomery.2@us.af.mil. Questions should be addressed to Primary POC-Miles Montgomery, Contract Specialist, USAF Academy, (719) 333-8093, louis.montgomery@us.af.mil NOT LATER THAN 10:00 AM MST on 18 SEPTEMBER 2019. Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be considered unless it is received before award is made, and the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition.
