THIS IS A REQUEST FOR INFORMATION (RFI) ONLY.
This is not a Request for Proposal (RFP) or Request for Quotation (RFQ).
In accordance with FAR 15.201, the Indian Health Service, Phoenix Indian Medical Center located at 4212 N. 16th Street, Phoenix, AZ 85016 is conducting a market survey to identify contractors the following: collection, transportation, and disposal of medical biohazardous and chemotherapy wastes accumulated by the Phoenix Indian Medical Center (PIMC) in Phoenix, Arizona and the Salt River Health Clinic in Scottsdale, Arizona. (Refer to attachment A for additional information.)
Interested contractors who meet the criteria above may submit information to the acquisitions department. This RFI is open from July 12, 2014 through July 21, 2014.
This RFI is for planning purposes only.
This sources sought notice/(RFI) is not to be construed as a commitment by the Government, nor will the Government pay for any information solicited.
