THIS IS A SOURCES SOUGHT NOTICE.
The purpose of this RFI is to obtain market information and capabilities for planning purposes and to determine appropriate strategies to meet the Agency's requirements. This RFI is issued solely for information and planning purposes and does not constitute a Request for Proposal (RFP), Request for Quote (RFQ) or a commitment for an RFP/ RFQ in the future. In accordance with FAR 15.201(e), Exchanges with Industry before Receipt of Proposals, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to the Sources Sought Notice. Information received from this Sources Sought Notice may be utilized in preparing potential solicitation documentation. It is the contractor's responsibility to identify proprietary information in response to this notice.
The purpose of this announcement is to gain knowledge of potential qualified sources and their size classification. NAICS Code 621991 applies.
The Central Texas Veterans Health Care System (CTVHCS) is seeking potential sources capable of providing blood and blood components, including but not limited to leukocyte, reduced red blood cells, fresh frozen plasmas, platelet pheresis, cryoprecipitate, and other blood components as necessary to meet our veterans' transfusion needs to the CTVHCS.
Potential contractors shall provide the capabilities to obtain blood and blood components, test said blood components to meet standards of safety as set forth by the FDA, and provide transportation of said blood and blood components and results to Central Texas Veterans Healthcare System as necessary to support our veterans' transfusion needs.
The potential contractors shall be capable of furnishing the above on a regular delivery basis (twice per week), extra delivery basis (coordinated extra weekly delivery), and STAT call basis (required delivery within 2 hours of notification). Supplies must be delivered to the CTVHCS, 1901 Veterans Memorial Drive, Temple, Texas 76504 in approved Styrofoam containers packed in ice.
The potential contractor shall have the capability to delivered blood units prior to the expiration date stated on each unit and certified that all units have been tested in accordance with the required mandates of the Food and Drug Administration (FDA) and the American Association of Blood Banks (AABB). Procedures for obtaining, storage, shipping and delivery must conform to FDA and AABB requirements. Potential contractor must certify that their blood center is currently licensed with the FDA and accredited by AABB. A copy of license and accreditation shall be submitted with your capability statement.
It is requested that contractor submit their capabilities responses to this sources sought notice. Contractor responses must include the following information:
1)    Company name
2)    Address
3)    Contact person name
4)    Contact phone number
5)    Email address
6)    DUNS number and cage code (Must be SAM registered)
7)    Socio-economic status (i.e. 8a, Hubzone, SDVOSB, VOSB, etc.)
8)    Comprehensive/detailed description of company's capabilities, to include origin of supplies proposed to be furnished, copies of all licenses/accreditations, and a list of sites providing similar supplies/services (please provide point of contact for customer)
