BLANKET PURCHASE AGREEMENT FOR VARIOUS ANNULOPLASTY RINGS FOR TRIPLER ARMY MEDICAL CENTER, HAWAII
This is a Sources Sought Notice (Request for Information). The Government intends to establish a multiple Blanket Purchase Agreement (BPA) for various annuloplasty rings for the Department of Surgery at Tripler Army Medical Center, Hawaii. The supplies are listed below; however it is not limited to these items:
(1) AnnuloFlex Mitral Flexible Repair Ring - Sizes 26mm - 36mm
(2) AnnuloFlo Mitral Repair Ring - Sizes 26mm - 36mm
The manufacturer is Sorin.
Calls will be placed against the BPA as supplies are required. Period of performance will be effective on or about 10 June 2014 for five (5) years. The NAICS code is 339112.
