This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in the Federal Acquisition Regulation (FAR 12.6) as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued.
Solicitation number 36C25619Q1050 is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005- 84.
The overall purpose is to provide ARC Flash Uniforms Leasing Service for the Southeast Louisiana Veterans Health Care System in New Orleans, LA.
Offerors shall provide business name, doing business as (DBA) name, mailing address, phone number, email, DUNS number, CAGE code and proof of active SAM registration when submitting offers in response to this solicitation.
When preparing offers in response to this solicitation, offerors shall refer to FAR 52.212-1 Instructions to Offerors-Commercial Items which provides specific guidance and instructions for offer submission. Offerors shall also familiarize themselves with 52.212-2 Evaluation-Commercial Items which provides the criteria for how offers will be evaluated.
