The DC VA Medical Center, Network Contracting Office (NCO) 5, Clarksburg, WV, is issuing a combined synopsis / solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is being issued. The solicitation number 36C24518Q0114 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 effective January 13, 2017. The NAICS code is 621910 and the size standard is $15 M. This requirement is unrestricted and will be fixed-price. The closing date for this notice is 1300 EDT March 30, 2018. The requirement is for the following:

The Contractor shall furnish all vehicles, personnel, management, equipment, supplies, services and facilities to provide Basic Life Support (BLS) unit and Advanced Life Support (ALS) unit ambulance service.. (See Performance Work Statement for additional detailed information.)

Contractor to provide services in accordance with terms and conditions of the contract for the period of April 1, 2018 through March 30, 2019.

To be eligible for award of a contract, vendor must be actively registered in the SAM database (https://www.sam.gov). Vendor is also required to complete representations and certifications found in the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items.
