AlohaCare, a nonprofit health plan which specializes in caring for those who are medically underserved and medically fragile, is accepting applications for the AlohaCare Community Conscience Award, a grant program specifically for Hawaii nonprofit organizations working to improve the health of Hawaii's communities.

(This award excludes community health centers, which AlohaCare is currently supporting through its quality improvement initiatives.)
Project must focus on health and wellness - educate others on the prevention and/or treatment of asthma, child and adolescent well-care visits and immunizations, diabetes, obesity, and/or women's health; access to care - reduce barriers to health care, such as lack of transportation, lack of health insurance, language or cultural barriers, and shortage of providers in rural areas; and/or aged, blind, and disabled populations - benefit the overall health and wellness of Hawaii's kupuna or those who are medically fragile and medically disadvantaged.
