Aging and Disability Resource Center Subgrants

The Department's mission in this request for proposal (RFP) is to identify a community with the capability to develop and implement an Aging and Disability Resource Center (ADRC). An ADRC project will serve as a highly visible and trusted place where individuals can receive information on the full range of long-term services and support options. An ADRC is a single entity coordinating information and access to public long-term services and support programs and benefits through various models of single or multiple points of entry.
