PROJECT TITLE: KANGAROO SLEEPER RECLINER
FOR VETERANS AFFAIRS SAN DIEGO HEALTHCARE SYSTEM

This is a combined synopsis/solicitation set-aside for Service Disable Veteran Owned Small Businesses (SDVOSB) and Veteran Owned Small Businesses (VOSB) for commercial supplies/services prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. The combined synopsis/solicitation is set-aside for qualified SB under NAICS 334118 (or related). This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes under solicitation number: 36C26219Q0207.

The Department of Veterans Affairs San Diego Healthcare System, San Diego, CA 92161 has a requirement for the Kangaroo Sleeper Recliners. The Contractor shall furnish all supplies/services at the VA San Diego Healthcare System 3350 La Jolla Village Drive, Warehouse Bldg. 1, San Diego, CA 92161.

GENERAL
VA San Diego has an Acute care unit that houses 10 patients and needs the recliners to be replaced. Due to the heavy use by our inpatient, damage to furniture is likely; we are seeking solutions that are built with safety and durability in mind. The purpose of this Statement of Work (SOW) is to outline the tasks and responsibilities of the successful offerors in relation to the VA San Diego Acute care installation, integration, and implementation of all furnishings. All counts and specific sizes are addressed in the salient specification attached.

SUBSTITUTION REQUESTS
Substitution requests ( OR EQUALS ) shall meet salient characteristics as outlined. Substitution requests shall be similar to the specified dimensions, color scheme, function and quality. Or equal products submissions shall include manufacturer s product brochure including specification information, warranty data, and physical samples of manufacturer s full range of finish options. FAILURE TO CLEARLY NOTE THAT THE PRODUCTS BEING QUOTED ARE THE REQUESTED BRAND NAME, OR FAILURE TO SUPPLY OR EQUAL PRODUCTS SPECIFICATIONS AND INFORMATION LITERATURE SHALL CONSTITUTE A NON-RESPONSIVE BID AND SHALL NOT BE FURTHER EVALUATED FOR AWARD.

INSPECTION AND ACCEPTANCE
The technical Point of Contact is responsible for performing inspections necessary to substantiate that the supplies furnished under this contract are not damaged and in good working condition. Any items found to be damaged and/or not in working condition, would be returned to the contractor and must be replaced in a timely manner.
WARRANTY REQUIREMENTS
All specified product(s) shall be under standard GSA Contract warranties.
Open Market items and specified fabrics shall meet the standard manufactures stated warranty claims as published.

DESIGN & CONSTRUCTION

Recliner


A wide back that supports our elderly patients with comfort, support and safety in mind. The chair shall lock in all its reclined positions so there is no sudden movement that would be potentially dangerous to our patients. The recliner should feature independent foot rest and back reclining motions. The back shall be reclined into an infinite number of positions. With a simple press of the foot pedal the recliner may easily be put into reverse recline position. Gliding sleeper as an option. Narrow poly arm grey arm cap with soft grip push bar.

Seat: Seat shall be composite foam with dynamic suspension system for long term comfort.

Applies to this acquisition and the following addenda to the provision.
(a) The Offeror s quotation shall be submitted electronically to the Points of Contact identified in the FBO combined synopsis/solicitation announcement by the date and time indicated.
(b) As part of your quote submission you are required to include price information. DUNS, Cage Code, tax ID, terms and conditions and Buy American Act Certificate required in clause 252.225-7000, as applicable.
(c) All firms must be registered in SAM at https://www.sam.gov at time of quote submittal in order to be considered for award. Please provide quote expiration date and best delivery schedule.


52.212-2(a) EVALUATION COMMERICAL ITEMS (OCT 2014)

Applies to this acquisition. Evaluation will be based on low price meeting brand name product or equal product IAW the outlined salient characteristics.
The Government reserves the right to award without discussions. Therefore, each initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if determined necessary by the Contracting Officer.


52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS COMMERCIAL ITEMS

The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u)) of this provision.
(a) Definitions. As used in this provision--
Economically disadvantaged women-owned small business (EDWOSB) concern means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. It automatically qualifies as a women-owned small business eligible under the WOSB Program.
Forced or indentured child labor means all work or service
(6) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or

Immediate owner means an entity, other than the offeror, that has direct control of the offeror. Place of manufacture means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.
Predecessor means an entity that is replaced by a successor and includes any predecessors of the predecessor.
Restricted business operations means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate
(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;
(2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization;

Service-disabled veteran-owned small business concern
(1) Means a small business concern
(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and
(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran.

Subsidiary means an entity in which more than 50 percent of the entity is owned

(2) Through another subsidiary of a parent corporation.
Successor means an entity that has replaced a predecessor by acquiring the assets and carrying out the affairs of the predecessor under a new name (often through acquisition or merger). The term successor does not include new offices/divisions of the same company or a company that only changes its name. The extent of the responsibility of the successor for the liabilities of the predecessor may vary, depending on State law and specific circumstances.
Veteran-owned small business concern means a small business concern
(1) Not less than 51 percent of which is owned by one or more veterans(as defined at 38 U.S.C. 101(2)) or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more veterans; and
(2) The management and daily business operations of which are controlled by one or more veterans.
Women-owned business concern means a concern which is at least 51 percent owned by one or more women; or in the case of any publicly owned business, at least 51 percent of the its stock is owned by one or more women; and whose management and daily business operations are controlled by one or more women.

(a) Definitions. As used in this clause-
(1) Contract financing payment has the meaning given in FAR 32.001 .
(2) Designated agency office has the meaning given in 5 CFR 1315.2(m).
(3) Electronic form means an automated system transmitting information electronically according to the accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, e-mail, and scanned documents are not acceptable electronic forms for submission of payment requests.
(4) Invoice payment has the meaning given in FAR 32.001 .
(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.
(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.

The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. The Government intends to award a firm-fixed-price contract. The Contract begins on the date of award. Due to time constraints, responses to the solicitation shall be due on Wednesday, December 19, 2018 at 11:00 AM PT.
