This Request for Information, Sources Sought/Market Survey Notice, is being conducted by the Network 10 Contracting office on behalf of the Aleda E. Lutz VA Medical Center (Saginaw VA Medical Center) located at 1500 Weiss, Saginaw, MI 48602. The purpose is to identify sources that can provide Full Maintenance and Repair Service for two (2) ABL80 Radiometer America Inc. Blood Gas Instruments: (SN: 309202, EE 6630-32369 and SN: 309187, EE 6630-32370) which are located at Aleda E. Lutz VA Medical Center (NAICS code 334516) Monday through Friday 8:00am EST - 4:30 pm EST, excluding federal holidays. All businesses eligible to provide this service are encouraged to reply as this information may be used to determine potential set-asides for the above noted service.

NO INFORMATION OTHER THAN THE COMPLETED VENDOR INFORMATION DOCUMENT WILL BE ACCEPTED.

STATEMENT OF WORK (SOW)

Contract Title. Full Maintenance and Repair Service for two ABL80 Radiometer America Inc. Blood Gas Instruments located at Aleda E. Lutz VAMC.
Scope: Contractor shall furnish all supervision, labor, travel, equipment, parts and supplies (excluding consumables) to furnish annual planned maintenance and any unplanned maintenance services for the ABL80 Radiometer America Inc. Blood Gas Instruments: (SN: 309202, EE 6630-32369 and SN: 309187, EE 6630-32370). Listed equipment is located at the Aleda E Lutz VA Medical Center, Laboratory Department, 1500 Weiss Street, room 1-2222, Saginaw, Michigan 40602. Service will be provided between 8:00am to 4:30pm, Monday through Friday, excluding Federal holidays.
Specific Tasks:
Contractor shall perform all manufacturer specified scheduled maintenance as published in the Manufacturer s Service Manual.

Scheduled Maintenance shall be performed at the Manufacturer s recommended frequency as published in the Manufacturer s Service Manual during the annual contract period.

Contractor shall furnish all supervision, labor, travel, equipment, parts, and supplies to accomplish the preventative or repair maintenance services during the annual contract period.

OEM parts shall be used for all maintenance and repair services.

Contractor shall provide a copy of the Service Report, either by email or by fax, no later than 6 work days after the completion of the scheduled maintenance or repair service.

The Service Report shall contain the following information:
Contract purchase order number
Date service initiated
Date completed
Equipment make and unique identification number/code
Description of original complaint
Problems found
Part/quantities installed
All work performed fully described

Performance Monitoring: Performance will be monitored by periodic inspections and observation of work. Work will be observed by the Contracting Officers Representative (COR), or designee, during performance of scheduled maintenance service and inspections and/or during repair service. Equipment operators will be contacted to report on contractor performance when COR, or designee, is not available.
Other Pertinent Information or Special Considerations. Only Manufacturer Trained Service Engineers/Technicians will perform services and repairs.
a) Inspection and Acceptance Criteria: COR, or designee, will verify that the unit is working within Manufacturer s specifications after completion of service or repair.
b) The Contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to any contractor representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the contractor s fault or negligence.
c) The Contractor will not discuss any patient issues that maybe observed/overheard during the course of the work. The Contractor will not have access to any patient data therefore is not required to engage in a Business Associate Agreement.
Delivery Schedule. Service Reports will be emailed or faxed to the COR (Nathan.Cox@va.gov) by the 6th workday after completion of work.
