Statement of Work
Signage for Baton Rouge South Outpatient Clinic

INTRODUCTION
These specifications describe the requirement for manufacturing and delivery of Room Number Signage for the Baton Rouge South Clinic located at 7850 Anselmo Lane, Baton Rouge, LA.
SCOPE OF WORK - OVERVIEW
This project shall consist primarily of manufacture and delivery of all Room Number Signage for the Baton Rouge South Clinic. The total number of signs to be manufactured is 115.
SPECIFICATIONS:
NOTE 1: All sizes and finishes must be verified with the COR before the final order is placed.
NOTE 1: THE SIZE PROVIDED ON ATTACHMENT 2 IS FOR BIDDING PURPOSES.
PRODUCTS/SALIENT CHARACTERISTCS: The product must meet all minimum salient characteristics defined in this section.
ALL VENDORS MUST PROVIDE A MOCK UP (6 tall x 9 ) FOR EVALUATION
CONSTRUCTION
Material shall be minimum 3/16 thick acrylic with white back spray and with raised black numbers and matching Braille letters. Typeface and font sizes shall match the photo (See Attachment 3).

DELIVERY AND INSTALLATION
DELIVERY
Delivery shall be coordinated with the SLVHCS COR, SHANE BRODINE at office phone 504-507-7036.
Contractor shall review all items needed for this project with the SLVHCS COR to ensure all items requested on the project are included.
Installation shall be by VA forces.
Delivery and unloading shall be at the loading docks off Tulane Ave. between South Galvez and Rocheblave. Two Service Elevators are available for delivery of products. Elevators may be shared with other contractors.
INSTALLATION
Installation of signage shall be by VA forces.
PERIOD OF PERFORMANCE AND WORKING HOURS
All signs must be manufactured and delivered no later than 6 weeks after NTP.
All deliveries shall be completed between 8:00 a.m. 4:00 p.m. Monday Friday. All federal holidays excluded. Delivery may be by mail or package delivery.
INSPECTION AND ACCEPTANCE
COR shall inspect the delivered product for general conformity at the time of delivery. As VA installs the product, the COR will verify quantities and product quality. The COR will notify the Contracting Officer of any discrepancies.
SECURITY REQUIREMENTS
SECURITY CLAUSE: "A&A requirements do not apply--Security Accreditation Package is not required".
GENERAL
Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security.
WARRANTY
The contractor shall provide a one-year manufacturer s warranty on all parts.
The warranty shall include all travel and shipping costs associated with any warranty repair.
