This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued.
The solicitation number is VA250-17-N-0778_1 and is issued as a Request For Quotation (RFQ)
The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR)
The North American Industry Classification System (NAICS) code is 339950, and the size standard is 500 employees.
Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/ and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the VIP at https://www.vip.vetbiz.gov as an SDVOSB or VOSB. A Contractor that proposes to provide a supply that is not manufactured by their organization (nonmanufacturer) shall meet the requirements of FAR 19.102(f).
Contractors that deem themselves capable of meeting the requirement shall provide the below information to Michael Iffland, Contract Specialist, at michael.iffland@va.gov no-later-than Wednesday, July 19, 2017 at 5:00PM, EDT. Responses shall include: (1) Business Name and Address, (2) GSA Contract Number, if applicable (3) Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size SMALL or LARGE (6) Type of Business: service disabled veteran owned, veteran owned small business, 8a, HUBZone, woman-owned, etc. (7) A concern that is not a manufacturer of the supply shall provide the manufacturers name and size standard. If the concern is the manufacture and the sole distributor please provide documentation to that effect.
Contractor must be registered with https://www.sam.gov
Supplies are to include only TAA compliant items. Items that are not TAA compliant must be clearly identified by the Contractor should the contractor choose to offer a quote.

Statement of Work

The facility seeks Patient Information Boards (PIBs) to be manufactured and installed by Vendor in patient rooms at the Ann Arbor VA Medical Center main building situated at 2215 Fuller Rd. Ann Arbor, MI 48105. The PIBs are for 6 South Inpatient Unit. They should have the following characteristics:
Constructed from tempered glass or other suitable, transparent material. Dry erase markers and permanent markers must completely wipe off board without damaging the surface. Must be magnetic. Samples of materials other than tempered glass should be provided to the CO prior to fabrication.
Frameless
Standoff mounting hardware
Inserts (sizes listed below in 5b) shall be printed on Durable Banner material with DuPont Tyvek or other waterproof material and will not streak or discolor if they become wet. Inserts must fit through the top of the boards and inserts must be easily replaceable by staff. Arial Font, minimum of 72 point shall be used for all text on the inserts.
(28) Patient Information Boards that are:
External size (24 W x 36 H x .25 Thick)
Ability to slide (18.75 W x 30.75 H) insert into erase board pocket without removing the board from the wall
(28) All Inserts will look like the layout in Attachment 1 (below). Should have a watermarked image of nature in the background, cannot be custom. Should have the same layout and all information provided as shown on attachment 1 (below).
(28) Glass Boards should be insert able and have a white frame border with the U.S. Department of Veterans Affairs logo in the lower right hand corner. These are to look like Attachment 2 (below).

52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price technically acceptable offer.
