DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK
DaTscan Ioflupane I123 Injection


1. Bidders shall provide all items as listed in the Supplies and Services Section of this solicitation. It is the intent of the Government to make a single award for Radiopharmaceuticals. Bidders are encouraged to submit a bid price for ALL line items indicated to be determined responsive.

2. Qualifications of Bidders: Bids will be considered only from bidders who are regularly established in the business called for and who in the judgment of the Contracting Officer are financially responsible and able to show evidence of the reliability, ability, experience, equipment, facilities and personnel directly employed or supervised by them to render prompt and satisfactory service.

3. Contractor/supplier shall abide by (CFR 10) Title Code of Federal Regulation, which states:

A License may use for medical use only:

(a) By-product material manufactured, labeled, packaged and distributed in accordance with a license issued pursuant to the regulations in Title 10, Code of Federal Regulations, Part 35 and the equivalent regulations of an Agreement State.

(b) Reagent kit that have been manufactured, labeled, packed and distributed in accordance with the approval by the Commission pursuant to S32.72 or an Agreement State under equivalent regulations for the preparation of radiopharmaceuticals for medical use.

4. Contractor shall have available an adequate supply of contract products to meet the requirements of the Michael E. DeBakey VAMC (MEDVAMC) at his/her distribution points.

5. Contractor shall label all supplies as to the amount of radiopharmaceutical, preparation time, expiration time, date, etc., and provide a bar labeled insert to allow scanning data into the current nuclear medicine information management system, Biodose.

6. All packaging of radioactive material MUST BE clearly labeled RADIOACTIVE on the outside of the shipping carton.

7. Contractor shall perform all quality control procedures as required by Federal and State regulatory agencies.

8. Contractor shall meet all Nuclear Regulatory Commission, Department of Transportation, F.D.A., OSHA and all other agency rules and regulations (Federal and State).

9. Contractor shall accept returns for credit under the following conditions:
a. Product(s) shipped in error.
b. Product(s) damaged in shipment
c. Concealed shipping damages
d. Recalled product(s)
e. Outdated products in unopened, original container (specifically authorized for return by manufacturer).

10. Contractor shall invoice once per month showing period covered, billing date, name of preparation, quantity and amount.

11. Ordering: After award, the successful bidder will be furnished the name(s) of individuals authorized to purchase against the Contract.
