The Canandaigua VA Medical Center located at 400 Fort Hill Avenue, Canandaigua NY 14424 is seeking interested Small Business Concerns and Other Than Small Business firms for a firm- fixed price contract to perform a Level II Historic American Buildings Survey (HABS).

THE GOVERNMENT IS UNDER NO OBLIGATION TO ISSUE A SOLICITATION AS A RESULT OF THIS ANNOUNCEMENT. RESPONSES REQUESTED FROM INTERESTED FIRMS:
Interested firms should indicate their interest in writing, as early as possible, but no later than October 22, 2019 with the following minimum information.

Indicate which set-aside you qualify for under NAICS code 541370 - Surveying and Mapping (except Geophysical) Services, Size Standard $15.0M: 8(a), HUBZone, SDVOSB, VOSB, WOSB, Small Business or Other Than Small Business.
A positive statement of your intention to submit an offer/bid when the solicitation is issued.
DUNS Number or Cage Code.
Provide evidence of recent experience (past 5 years) in work similar in type and scope as the Prime contractor. For each contract/project submitted, include the Contract Number, Project Title and Description, Award and Completion Dates, Dollar Value of Contract, Points of Contacts for each contract to include current telephone numbers and email addresses.

SCOPE OF WORK

DESCRIPTION

In order to implement Stipulation I of the Memorandum of Agreement on construction of CLC cottages in Canandaigua, New York (attached hereto), the Canandaigua Department of Veterans Affairs (VA) Medical Center requires the services of a professional historic preservation firm ( The Contractor ) to perform Level II Historic American Buildings Survey (HABS) documentation with respect to:

Canandaigua Veterans Administration Medical center (VAMC)
Canandaigua VAMC Neuropsychiatric Infirmary Bldg. No. 33
And Canandaigua VAMC Neuropsychiatric Infirmary Bldg. No. 34; all located at 400 Fort Hill Ave., Canandaigua, 14424 NY

2.0 GENERAL

The Contractor shall provide all labor, supervision and materials necessary to perform the task as indicated in the scope of work.

All work is to be performed during normal business hours. If The Contractor anticipates that an interruption in the flow of work will occur, approval from the COR must be obtained prior to the interruption.

The Government will not be held responsible for bidding errors made or the failure to adequately inspect the job site for existing conditions. All bidders are strongly encouraged to inspect the job site prior to submitting bids.

Any existing feature disturbed (damaged, etc.) by The Contractor during the performance of this contract shall be restored to similar or better condition by The Contractor at the contractors expense.

During the entire duration of work performed, The Contractor shall, at all times, maintain at least one lane for emergency traffic.

The VAMC Project COR shall govern all work done during project duration. Disputes shall be promptly referred to the Contracting Officer, (CO). The Contractor will immediately correct any work in which the Project COR deems unacceptable or not in accordance with plans and/or specifications.

The Contractor is responsible for site restoration of the area damaged from any construction activity immediately including exterior site, hallways, stairwells, and roof material.

The Contractor will not be allowed to stage material and/or equipment outside of the designated area. All material and/or equipment will be stored in an approved location by the COR.

Schedule of Documentation for the recording of Canandaigua Veterans Administration Hospital, Canandaigua VA Hospital Neuropsychiatric Infirmary Building No. 33 and Canandaigua VA Hospital Neuropsychiatric Infirmary Building No. 34 shall include:

Written documentation
Measure drawings (Building 33 and 34)
Graphic Documentation
Photographic Documentation
Electronic Copy

DELIVERABLES

The Contractor shall provide one (1) complete package for submission to the National Park Service. The Contractor shall organize this project as three separate parts the way they are set out in the three schedules of documentation (see attached). Each part shall have a cover sheet, written report, site maps and/or drawings/plans, photographs, index and key to photographs.


Deliverables must follow guidelines provided by the National Park Service - Transmittal, Photo Guidelines and History Guidelines (see above section 3.0 Documentation), as well as the guidelines attached in Schedule of Documentation from the U. S. Department of the Interior. Deliverables have to include, but are not limited to:

Photography:
Prepare and provide not more than 100 or less than 20 original photographs (ca. 30 shots per building/structure), including views of the overall campus, exterior and interior views of buildings 33, 34 and connecting tunnels, as well as images of any architectural details judged by the contractor, VA and the National Park Service to be worthy of documentation. Photography shall include, but not limited to:
One (1) 4 x 5 black-and-white archival negative per image;
One (1) 4 x 5 black-and-white archival contact print per image, one each on an archival mount card;
One (1) photo-caption index containing building name, subject, and view; and
Digital versions of all images and index on an archival compact disc (CD) or Digital Versatile Disc (DVD).
All negatives and prints must be delivered housed in 4 x 5 archival sleeves/envelopes labeled to correspond with a photo-caption index. Only the 4 x5 format is acceptable (no smaller or larger).

Drawings:

One (1) copy of each drawing, readily reproducible, ink on translucent, archivally stable materials in 19x24 or 24x36 sizes;
One (1) reduced scale (8.5x11 ) copy of each drawing; and
Digital versions of all drawings on archival CD or DVD.

Narrative - Written History and Description:

One (1) copy of the report and any supporting document, readily reproducible, on archival bond 8.5x11 paper.
Digital versions of report and documentation on an archival CD or DVD.

The Contractor shall also provide one (1) regular CD or DVD with an entire submission to VA FPO, one copy (1) to Sonnenberg Gardens Museum and one copy (1) to NY SHPO.

5.0 INCLUSIVE CONTRACT

The contract will include all services, supplies, and costs associated with travel, transportation, and insurance.

6.0 SUBMISSION

All deliverables must meet the standards established by the Historic American Buildings Survey/Historic American Engineering Record/Historic American Landscape Survey (HABS/HAER/HALS), National Park Service, Department of the Interior.

The submission will be subject to VA s approval in consultation with Northeast Region of the National Park Service. The Contractor will ensure final acceptance by The U.S. Department of the Interior, National Park Service, Heritage Documentation Programs, Historic American Buildings Survey, 1201 Eye Street, NW, Washington DC 20005. The submission will be filed with the HABS/HAER/HALS collection at the Library of Congress.
