**Please read everything before submitting bids as there have been changes that have been made from past requirements. Failure to submit bids fully filled out or with required documentation as required WILL NOT be considered for bid. Please submit certifications with kosher bids showing the items that are being bid on. Must return a signed copy of the RFQ solicitation and have page 5 filled out. Please include on bid phone number, fax number and email. Freight MUST be included. If there is a minimum ammount it must be included on the bid sheet. **
